Our clients include brands you know like Microsoft, Target, and Delta Airlines, and we’ve earned external validation of net savings for customers.</p><p>We’re seeking an independently licensed mental health professional to join the Care Navigation team (LCSW, LMFT, LPC required) to support members seeking care with chronic and treatment-resistant mood and anxiety disorders. The Mood and Anxiety Specialty Care Navigator will provide initial assessment, support and coaching, referral, and ongoing care coordination for members within Spring Health’s Mood and Anxiety Disorders Program. This is a full-time, fully remote role with a Monday–Friday, 9–5pm EST schedule.</p><p><b>The following sections outline responsibilities, qualifications, and benefits.</b></p><h2>What you’ll do</h2><ul><li>Telephonic screening, brief intervention, and referral of members to mood- and/or anxiety-focused specialty treatment providers.</li><li>Apply active listening, psychoeducation, and motivational interviewing to understand member needs and support goal setting.</li><li>Coach members using DBT skills as part of their care journey and assist with engagement in care through motivational interviewing.</li><li>Identify appropriate level of care, facilitate referral, and coordinate care with a Spring Health provider.</li><li>Provide consultation on mental health concerns and family/relationship issues as appropriate.</li><li>Offer in-the-moment support and safety planning for members with safety risks or crises when needed.</li><li>Communicate with members and providers via written communication and maintain accurate records within required timeframes in an EHR system.</li><li>Identify and resolve barriers to care and collaborate with multi-disciplinary teams to ensure a smooth member experience.</li><li>Support other specialty care teams (SUD, Trauma, Eating Disorders) as needed and participate in staff meetings, case consultations, and trainings.</li><li>Assist with onboarding and mentoring new team members while upholding confidentiality, compliance, and ethical standards.</li></ul><h2>What success looks like</h2><ul><li>Maintain high member satisfaction as measured by survey responses.</li><li>Retain members throughout their course of care through proactive outreach and engagement.</li><li>Triage and enroll members into appropriate levels of care based on presenting problems and goals.</li><li>Proactively outreach to schedule triages.</li></ul><h2>What you’ll bring</h2><ul><li>Required mental health license: LCSW, LMFT, LPC.</li><li>Minimum of 2+ years of clinical care experience with chronic and severe mood and anxiety disorders.</li><li>Experience with motivational interviewing, diagnostic assessment, and triage.</li><li>Knowledge of best practices and evidence-based treatments for mood and anxiety disorders; DBT experience/training preferred.</li><li>Excellent listening, written, and verbal communication skills.</li><li>Commitment to diversity and inclusion; experience working with diverse populations.</li><li>Tech-savvy and comfortable learning new systems; ability to multi-task and switch contexts.</li><li>DBT certifications preferred.</li></ul><h2>Compensation and benefits</h2><p>Target base salary range: $33.33 - $42.31 hourly, with a competitive total rewards package including stock options and benefits.
