Registered Nurse - Case Manager (Travel)

Location: Santa Clara, CA
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5×8 Days
Pay: $2,948/week
Completion Bonus: $2,500 per contract (up to $7,500 after three contracts)

Position Overview

ATC Healthcare is seeking an experienced RN Case Manager for a hospital-based travel assignment in Santa Clara, California. The RN Case Manager will coordinate patient care across the continuum, ensuring appropriate resource utilization, discharge planning, and compliance with hospital and regulatory guidelines. This role supports patients through comprehensive assessment, care coordination, and transition planning to promote continuity, quality outcomes, and cost-effective care.

Key Responsibilities
  • Perform comprehensive assessments to identify patient needs and barriers to care.
  • Develop, coordinate, and implement individualized care and discharge plans.
  • Ensure appropriate level-of-care placement and conduct utilization review in alignment with payer requirements.
  • Serve as a liaison between the interdisciplinary care team, patient, and family.
  • Support patients through their inpatient stay, providing education and care coordination.
  • Ensure all documentation is accurate and compliant with hospital, state, and regulatory standards.
  • Advocate for patient needs while ensuring safe and timely discharge planning.
  • Collaborate with external agencies and community partners to ensure continuity of care.

Qualifications
  • Active California RN License (required).
  • BLS (AHA) certification (required).
  • No formal qualification assessment required (skills-based hiring accepted).
  • Experience in care coordination, discharge planning, or case management in an acute care hospital (preferred).
  • Strong understanding of clinical workflows, utilization review, and care transition processes.
  • Excellent communication, critical thinking, and organizational skills.
  • Ability to work an 8-hour day shift Monday-Friday.
  • Proof of COVID-19 vaccination and recent tuberculosis test required.
