Position Summary Working under the direction of the Director of Nursing (DON), the MDS Coordinator is responsible for conducting and coordinating the development and completion of the resident assessment in accordance with applicable regulations and facility policies and procedures. In addition, this position is responsible for developing care plans for each resident and directing members of the interdisciplinary team (IDT) to complete certain aspects of the care plans. The MDS Coordinator also, under the direction of the DON, routinely audits resident charts to ensure various requirements are being met and provides education to team members on applicable topics (e.g. care planning, point of care documentation, etc.) on an as-needed basis. Primary Responsibilities • Conduct and coordinate the development of resident assessment (MDS) in accordance with current rules, regulations and guidelines that govern the resident assessment. • Perform administrative duties, including electronic record administration, medical forms, reports, evaluations, studies, etc. as necessary. • Develop, implement and maintain ongoing quality assurance program for the resident assessment/care plans. • Participate in facility survey (inspections) made by authorized government agencies. • Lead/Direct the interdisciplinary care planning process and work with the team in developing a comprehensive resident assessment and care plan for each resident. • Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment. Develop and implement procedures with the Director of Nursing to inform all assessment team members of the arrival of newly admitted residents. • Collaborate with the IDT to ensure all needed clinical documentation is in place for each resident. • Complete applicable audits and present findings. • Participate in all relevant Health Care Center and facility-wide in-service training programs. • Actively participate in the facility's Quality Assessment & Assurance (QAA)/Quality Assurance and Performance Improvement (QAPI) program. In certain cases, lead/direct performance improvement efforts as assigned. • Maintain and ensure compliance with resident rights, including the confidentiality of all resident care information. • Provide training/education to other staff members on relevant subjects (e.g. MDS interviews and coding). • Perform other duties as assigned. Skills • Strong background in medical office procedures, particularly within inpatient settings. • Experience with discharge planning processes and familiarity with home care services. • Knowledge of working within a Level II Trauma Center environment is preferred. • Proficiency in electronic health record systems; experience with eClinicalWorks is a plus. • Ability to work collaboratively within a clinic or hospital setting, demonstrating excellent communication skills. • Experience in primary care or pediatrics is advantageous. • Strong organizational skills with attention to detail and the ability to manage multiple tasks effectively. Minimum Qualifications • Current, unencumbered license to practice as a Registered Nurse in California. • Experience as a MDS Coordinator, preferred. • At least two years of experience in a LTC - California-based skilled nursing facility. • Current knowledge/understanding of nursing and medical practices and procedures, as well as relevant laws, regulations and guidelines that pertain to long-term care. • The ability to deal tactfully with personnel, residents, family members, visitors, government agencies and the general public. • Must demonstrate excellent skills in reading writing, speaking and understanding the English language. Other languages a plus. • Excellent communication, decision-making and interpersonal/communication skills. • bolthires office and computer proficiency.
