Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state, and the largest provider of in-home attendant care services. Current hourly rate for this position includes $3 per hour in HERO Pay. This HERO Pay will last for a minimum of 6 months past the end of the Federal Covid Pandemic. At that time HERO Pay will end, and hourly rates will reduce by $3 per hour to a normal base rate. Under the supervision of the Home Care Coordinator, the Attendant Care Associate is responsible for providing and assisting participants with activities of daily living, restorative and supportive care. Assists with activities within the home to maintain the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the participant.
Position Responsibilities:

  • status, changes in plan of care and to confirm scheduled visits.
  • Under the direction and supervision of the Home Care Registered Nurse assists participants with the following duties:

o Personal hygiene such as skin, mouth, hair care and bathingo Assists participants with ambulationo Assists participants with meals/Prepare mealso Assist participants to the bathroom or in using commodes, bedpans, or urinals * Reports changes in participant’s condition and needs to the Home Care Director (or designee)

  • Light housekeeping
  • Meal Preparation
  • Transport or escort for essential errands and medical appointment as needed
  • Completes appropriate documentation of services provided following Horizon program policy
  • Follows all Horizon program policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications
  • Participates in and support Quality Improvement Initiatives
  • Performs other duties as required or requested

  • Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment

  • Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects
  • Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances
