RN Case Manager Position Summary

  • Performs patient assessments based on agency standards of practice to ensures effective and appropriate home health care services.
  • Directs, plans and initiates appropriate action independently and responsively in home health care situations.
  • Supervises LVN's, home health aides and personal care aides to provide effective and quality home health care services.
  • Completes assigned tasks within established guidelines and time frames.
  • Documents OASIS assessments accurately in the system within the given time frames.

Qualifications:

  • Current California RN License
  • Friendly/ Excellent customer service and great bed side manners
  • Good Listener
  • Wound Care experience or certification
  • Minimum of one year experience as an RN

Pay varies based upon home health experience and documentation timeliness. Annualized pay estimated from $111,000 per year and up.

For LVNs: Annualized pay estimated at $76,500 per year and up.
