And we do it all with heart, each and every day.</p><p><b>Position Title:</b> Registered Nurse – Transform Diabetes Care Pod</p><p><b>Position Summary:</b></p><p>The Transform Diabetes Care Pod RN plays a vital role in improving member health outcomes by engaging telephonically with individuals living with diabetes. This nurse will guide members through actionable steps to close gaps in care, assess medical needs, and support overall wellness. A key focus of the role is addressing social determinants of health that contribute to care gaps, while providing education, connecting members to resources, and facilitating referrals to care management programs for continued support.</p><p>This position supports the development and execution of health services strategies and programs that promote high-quality, cost-effective care. Using advanced clinical judgment and critical thinking, the RN coordinates physical and behavioral health services, collaborates with providers, and ensures access to psychosocial support services. The goal is to optimize resource utilization and drive positive health outcomes.</p><p>The RN will use strong interviewing and engagement techniques to assess member health status and needs, while adhering to regulatory and organizational policies. Collaboration with supervisors and team members is encouraged to overcome barriers and achieve program goals.</p><h2>Responsibilities</h2><ul><li>Conduct telephonic outreach to members with diabetes to assess needs and close gaps in care.</li><li>Provide education and support to promote member engagement and self-management.</li><li>Identify and address social determinants of health impacting care.</li><li>Refer members to appropriate care management programs and community resources.</li><li>Collaborate with providers and interdisciplinary teams to coordinate care.</li><li>Apply nursing processes in compliance with company and regulatory standards.</li><li>Document interactions and care plans accurately in electronic medical records.</li><li>Consult with supervisors to resolve challenges and improve member outcomes.</li></ul><h2>Required Qualifications</h2><ul><li>Minimum 3 years of clinical experience supporting individuals with chronic conditions.</li><li>Diabetes education experience is required.</li><li>Current, unrestricted Registered Nurse (RN) license; multi-state licensure preferred.</li><li>CDE or CDCES certification required.</li><li>Proficiency in Microsoft Office Suite (Outlook, Excel), EMR systems, and keyboarding.</li></ul><h2>Preferred Qualifications</h2><ul><li>Bilingual (Spanish) preferred.</li><li>Strong communication and interpersonal skills.</li><li>Ability to engage members effectively and assess health needs through conversation.</li></ul><h2>Education</h2><ul><li>Associates Degree required. Prefer Bachelor of Science in Nursing (BSN).</li></ul><h2>Anticipated Weekly Hours</h2><p>Full time</p><h2>Pay</h2><p><b>60,522.00 - $129,615.00</b></p><p>This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
