**Job Description** The Essex County Special Child Health Services Case Management (SCHS CM) Unit is seeking a Case Manager to support families of Children and Youth with Special Health Care Needs (CYSHCN), ages birth to 22. The role involves a team-based approach to: - Assess and coordinate comprehensive services (medical, developmental, educational, and community-based) - Serve as a liaison between families, providers (PCPs, specialists, schools, home care agencies), and community resources - Maintain accurate case records, daily logs, and essential documentation - Facilitate culturally and linguistically appropriate communication and team meetings - Monitor service outcomes, advocate for families, and help them access cost-effective care **Requirements and Qualifications** - Bachelor’s or Master’s degree in Social Work (BSW/MSW) or Bachelor of Science in Nursing (BSN) - Minimum one year of pediatric case management experience with CYSHCN - Knowledge of medical and developmental disabilities, infant/early childhood development - Valid New Jersey driver’s license (if required for travel to perform duties) - Proficiency in Microsoft Office and electronic/manual record-keeping systems - Excellent verbal and written communication skills; culturally sensitive and professional demeanor - Ability to organize work, set priorities, work independently, and collaborate effectively with families and community agencies **Pay and Benefits** - Salary: Commensurate with education and experience (typically in the range of $ Fifty-two thousand to $Sixty-five thousand annually for entry-level social work positions in Essex County) - Comprehensive health, dental, and vision insurance - Public employee pension plan and optional deferred compensation (457) - Generous paid time off (vacation, sick leave, personal days) - Tuition reimbursement and professional development opportunities - Employee assistance program, wellness initiatives, and life insurance **Company Information** Essex County Government, located in Newark, New Jersey, serves a diverse population of roughly 800,000 residents across 22 municipalities.
